Welcome to Naval Health Clinic Lemoore Medical Home Port! The Medical Home Model is an approach to healthcare delivery, based on the concept that patients are at the center of every decision made regarding their care. The aim of this model is to provide integrated patient centered care that is also high quality and accessible. Your Medical Home Port (MHP) team will coordinate with you to provide the best possible evidence based care to fulfill your personal healthcare needs.
Your Medical Home Port healthcare team will consist of:
- Primary Care Manager (PCM)
- Register Nurse (RN)
- Licensed Vocational Nurse (LVN)
- Hospital Corpsmen/Medical Assistant
We provide routine and preventative care services for active duty, active duty family members and retires. Active duty assigned to the operations side should see their PCM at the Hornet clinic for any routine or acute issues. We also have an Integrated Behavioral Healthcare Consultant and Clinical Pharmacist available to address your healthcare needs
Enroll on a Team:
To enroll on a medical home port team please call Health Net (TRICARE) 844-866-9378. Upon completion of enrollment, you will be assigned a MHP Team, either
Blue or Gold.
Meet your team:
We have a team of providers including Family Physicians, Nurse Practitioners and Physician Assistants.
Make an Appointment:
In order to make an appointment with your PCM please call the appointment line at (559) 998-0889, select option #1. We will make every effort to schedule you with your PCM, however if we are unable to, you will be scheduled with another provider on your team. If you need to cancel your appointment please call (559) 998-0889, select option #1 in order to speak with a MHP team member.
Please make every effort to cancel your scheduled appointment time within 24 hours or as soon as possible, in order to have that time slot available for another patient.
Managing your Health Information:
You can access your own personal health information including provider visits, medications, laboratory and radiology results at
MHS Genesis Patient Portal. If your provider places a referral for specialty services you can see the status of the referral and get more information about your care at
www.tricare-west.com or by calling 1(844) 866-9378.
