Deployment Health Center  

The Deployment Health Center (DHC) is here to provide health screening and services which span the full deployment cycle.

Individual Medical Readiness (IMR; Annual Birth Month Periodic Health Assessment) that includes: 

  • Periodic Health Assessment (PHA)
  • Immunization Status Monitoring
  • Readiness Laboratory Studies (DNA, ABO blood typing, Sickle Cell, and HIV)
  • Individual Medical Equipment Monitoring (glasses, contact lenses, gas mask inserts, hearing protection) 

Individual Augmentee Screening

  • Pre-Deployment Health Assessment
  • Post-Deployment Health Assessment
  • Post-Deployment Health Reassessment (PDHRA; required 90-180 days post deployment)

Sea/Overseas Suitability Screening

The Walk-in Contraceptive Services Clinic is held every Thursday from 7:30 - 11 a.m. at the main side OB/GYN window at Naval Health Clinic Lemoore.  First come, first served.
